Community Partnerships

We create onsite research supported, evidence based programs that provide care coordination to address health disparities. Our efforts reduce the over utilization of emergency room services, ambulance services, and avoidable hospitalization. AHS helps residents navigate the health care system by reducing the fragmentation of healthcare services in lower income communities.
